Mobile Passport: Emergency Sheet Export

Edited

The Mobile Passport is a one-click emergency summary for a participant. If someone needs to go to the hospital, you can print it or hand it to EMTs so they know who the person is, what medications they take and what has happened with them recently.

What's included

  • Face Sheet information: the participant's details as set up on your Face Sheet, such as name, date of birth, photo, emergency contacts, physician and allergies.

  • All active medications from the participant's Medical Info.

  • The last 48 hours of actions recorded for the participant in StoriiCare, so hospital staff can see recent care, not just a static profile.

Like every StoriiCare export, the Mobile Passport also shows your business name and branding, and the participant's profile picture if one has been uploaded.

How to export a Mobile Passport

  1. From the Dashboard, open Exports.

  2. Select Mobile Passport.

  3. Choose the participant.

  4. The Mobile Passport opens as a PDF in a new window. Print it, or download it to send with the participant.

Get it ready before you need it

The Mobile Passport is only as good as the information behind it. A few minutes of setup makes it far more useful in an emergency:

  • Put emergency details on the Face Sheet. Allergies, seizure or other high-risk information, physician, emergency contacts and any guardian or conservator should all be filled in. See How to Edit the Face Sheet Template.

  • Keep medications current. Add new medications as they are prescribed and remove ones that have stopped, so only active medications appear. See How to Add Medications.

  • Record care as it happens. Notes, vitals and medication records entered during the day are what make the 48-hour history useful.

  • Show staff where it is. Make sure everyone who might deal with an emergency, including drivers and floor staff, knows how to pull a Mobile Passport.

Handling it safely

A Mobile Passport contains protected health information (PHI). Share it only with the people treating the participant, such as EMTs or hospital staff. Don't send it by regular unencrypted email, and shred printed copies you no longer need.

If StoriiCare can't be reached

StoriiCare needs an internet connection; it doesn't store participant records on your devices, which keeps PHI off phones and tablets that could be lost. Plan for an outage the same way you would for any system. See StoriiCare Backup Documents and When StoriiCare Is Not Loading.
