Choosing How to Document Care Plans on StoriiCare
StoriiCare offers two primary options for documenting and managing Care Plans:
The Care Plan feature (sometimes referred to as Support Plan, Service Plan, Know Me Better, etc. — depending on your organization’s terminology), and
Forms, created and customized through Form Builder.
Both options are powerful and customizable. The right choice for your organization depends on your care documentation requirements and workflow preferences.
Option 1: Using the Care Plan Feature
The Care Plan feature is ideal for organizations that want a structured, interactive, and easy-to-navigate way to create and review care plans.
You might prefer the Care Plan feature if:
You want to link goals, outcomes, KPIs, actions, or responses directly to each plan.
It’s important to organize care plans by category (e.g., Physical Health, Emotional Wellbeing, Daily Living, etc.).
You value a clean, simple, and user-friendly interface for staff.
You want to archive (“mark as historical”) old care plans while maintaining visibility.
You’d like to see a trail of update notes added by staff over time (terminology is customizable).
Customization & Configuration
The Care Plan feature template can be customized under Business Settings > Care Plans.
You can:
Add or rename plan categories
Create custom instructions, section prompts, and fields
Make fields required or optional
Add dropdowns and date fields
Link care plans to tasks for review reminders
Upload additional documentation

Option 2: Building Care Plans Using Forms
Some providers choose to document their Care Plans using Forms built in the Form Builder tool. This option allows for more control over structure and layout — especially when your organization must meet external documentation standards.
You might prefer Forms if:
Your state, region, or governing body requires a specific form, PDF, or template.
You need to capture digital signatures (these can only be added within Forms).
You must display specific participant data points (e.g., Face Sheet fields such as ID Number, Admission Date, Allergies, and Emergency Contact).
Your Care Plan includes an assessment or scoring element that requires a Matrix Calculator.
Customization & Configuration
Forms can be built and edited through Form Builder, allowing you to:
Add signature boxes, formulas, and conditional logic
Pull through Face Sheet fields automatically
Set required fields and review dates
Link completed forms to tasks and workflows
Upload supporting documents
Notify management when a plan is created or edited
Shared Functionality Between Care Plans & Forms
Regardless of which option you choose, both the Care Plan feature and Forms include:
Extensive customization through Form Builder or Business Settings
Ability to add custom categories, instructions, and dropdown menus
Integration with Tasks for setting review reminders
Options to upload attachments or supporting files
Workflows that can notify management when a plan is completed or updated
Fields to enter review dates and mark sections as required
Understanding Historical Views & Versioning
StoriiCare currently offers limited historical tracking for Care Plans and Forms.
Here’s how each feature handles versioning and updates:
When using Forms:
There is no automatic version history.
To review or update a plan, users must duplicate the existing form, make edits, and retain the previous version as a record. Previous versions can be marked as complete to prevent any further edits from being made to that form.
When using the Care Plan feature:
You can archive (“mark as historical”) old care plans.
If you edit an existing plan, the previous version is overwritten.
Alternatively, you can leave the existing plan unchanged and record updates in an “Update Notes” section (terminology customizable).
Final Tip
Many providers use both features strategically:
The Care Plan feature for everyday planning and tracking progress, and
Forms for compliance-based or regulatory documentation.
