How to Link Care Plans & Outcomes
StoriiCare enables care providers to link Care Plans with Outcomes (also referred to as Goals, KPIs, Actions, or Responses) to create a more connected and measurable care framework. Linking these two features helps connect related elements and ensures care delivery aligns with the individual’s personal goals and needs.
📝 Care Plan may appear as Support Plan, Service Plan, Care & Support Plan, Know Me Better, etc., depending on your organization’s terminology.
📝 Outcomes may appear as Goals, Objectives, Key Performance Indicators, Responses, Actions, etc., depending on your organization's terminology.
Why Link Care Plans & Outcomes?
Linking Outcome within a Care Plan allows you to:
Demonstrate how daily care activities support specific outcomes or goals.
Measure progress and effectiveness over time.
Keep care documentation clear, consistent, and outcome-focused.
Provide evidence for regulatory or quality reporting.
Getting Set Up
Linking an Outcome to a Care Plan involves two prerequisites:
Outcomes must already be created within the Outcomes feature.
An Outcome selector dropdown must be present in the Care Plan. This can be via Business Settings > Care Plan. From here, you can also make this field required and decide whether it should be a single-select or multi-select dropdown.
👉 Setting Up the Care Plan Template
Step 1: Open a Participant’s Profile
From your StoriiCare dashboard, locate and click on the participant’s name.
In their profile, navigate to the Care Plan feature.
Step 2: Select a Care Plan
Choose the Care Plan category (for example, Mobility & Dexterity or Personal Care).
Click the arrow next to the Care Plan you want to link.
This will expand the Care Plan so you can view its details.
Step 3: Edit the Care Plan
In the top-right corner of the expanded Care Plan, click the settings/cog wheel (⚙️).
Select Edit from the dropdown menu.
If you do not see the Edit option, you may not have permission to make changes. Contact your manager or administrator for access.
Step 4: Link to an Outcome
Within the Care Plan edit view, look for a section labeled Linked Outcomes, Associated Goals, or Related Actions (terminology may vary based on your organization’s setup).
Click the dropdown menu or link icon to open the available outcomes.
Select the relevant Outcome/Goal/KPI from the list.
You can link multiple outcomes to a single Care Plan if needed.
Once the appropriate outcomes are selected, click Save at the bottom of the page.
Step 5: Viewing Linked Outcomes
After saving your changes:
Linked Outcomes will appear directly within the Care Plan view for quick reference.
Staff can click on a linked Outcome to view progress updates, review the Outcome, or record notes (relative to their permissions).
This creates a clear connection between care delivery (the Care Plan) and goal achievement (the Outcome).
Your Care Plan will now display the linked Outcomes under the plan details.
