How to Delete a Care Plan

If a Care Plan was created by mistake or is no longer needed, you can delete it directly from the participant’s profile. Please note that deleting a Care Plan permanently removes it from StoriiCare — unlike archiving or marking a care plan as historical, deleted plans cannot be recovered.

⚠️ Important: Before deleting a Care Plan, ensure your organization’s policy permits it. In most cases, archiving (marking as historical) is preferred over deletion for compliance and recordkeeping purposes.


Step 1: Open the Participant’s Profile

  1. From your StoriiCare dashboard, find the participant whose Care Plan you want to delete.

  2. Click their name to open their profile.

Step 2: Access the Care Plan Feature

  1. Within the participant’s profile, click the Care Plan feature.

    • Depending on your organization’s configuration, this may appear as Support Plan, Service Plan, Know Me Better, or similar.

  2. You’ll see a list of Care Plan categories.

Step 3: Select the Care Plan

  1. Click on the category tile that contains the Care Plan you wish to delete.

  2. Find the specific Care Plan and click the arrow next to it to expand it fully.

Step 4: Delete the Care Plan

  1. In the top-right corner of the expanded Care Plan, click the settings/cog wheel (⚙️).

  2. From the dropdown menu, select Delete.

  3. A confirmation message will appear asking you to confirm the deletion.

  4. Click Yes to permanently delete the Care Plan.

🔐 Note: If you do not see the Delete option, your account may not have the required permissions. Contact your manager, administrator, or supervisor for assistance.

Best Practice: Use “Mark as Historical” Instead

Whenever possible, use the Mark as Historical option instead of deleting.

This allows your organization to:

  • Maintain a record of past Care Plans for compliance and auditing.

  • Track the participant’s care progression over time.

  • Avoid accidentally losing important care information.
