Using Checkboxes in Forms

Checkboxes are an excellent choice when you need users to select multiple options or “check all that apply” within a form.

Customizing the Checkbox Component

To add and configure checkboxes in your form, follow these steps:

1. Add a Checkbox: Drag the Checkbox component from the Form Builder toolbar onto the canvas.

2. Open Settings: Click the settings wheel icon on the checkbox to open the Options panel.

3. Set the Label: Enter a descriptive Label for your checkbox.

4. Customize the Selection Symbol: By default, the selection symbol is a check mark (✔), but you can use the Selection Glyph option to choose a different symbol if needed.

5. Adjust the Color: The default symbol color matches your account’s primary color (as set in Center Settings). To override this, select black, red, or blue from the Selection Color dropdown.

6. Pre-Select Boxes (Optional): If you’d like any checkboxes to be selected by default, check the box labeled Is checked.

7. Matrix Score (Optional): If you are building a scored assessment, this is where you can apply the numerical value to each checkbox. (Learn more about creating a Point-Based assessment here)

8. Duplicate for Efficiency: Instead of dragging multiple checkbox components individually, use the duplicate icon (two overlapping squares) to quickly create additional checkboxes and edit their labels.
