How to Add Medical Conditions
The Medical Conditions feature in StoriiCare allows care teams to document a participant’s diagnosed medical conditions in a clear, structured way. Keeping this information accurate and up to date supports safe care delivery, informed decision-making, and effective communication across teams.
Medical Conditions are recorded within a participant’s Medical Info section and are visible to authorized staff.
Important Clinical & Documentation Guidance
When adding medical conditions:
Record diagnosed or clinically confirmed conditions only
Use clear, accurate, and unambiguous descriptions
Review how conditions display after saving to ensure clarity
Review medical conditions regularly during care plan reviews
Medical Conditions should be treated as part of the participant’s clinical record and maintained accordingly.
🧠 Tip: Medical Conditions is a linked field type and can automatically display on a participant's Face Sheet.
How to Add a Medical Condition for a Participant
Open a Participant’s profile.
Click Medical Info.
Select Medical Conditions.
Click + Add New Condition.
Type in the Condition Name (required).
Enter the Date of Diagnoses (optional).
Enter Individual Specific Information (optional). This might be something like severity of the condition, typical symptoms for the individual, management considerations, etc.
Enter Generic Condition Information (optional).
Click Save new medical condition when finished.
