Care Plan Reviews: How to Schedule & Complete Them
Regular Care Plan reviews are an important part of maintaining accurate, up-to-date records of each participant’s needs, goals, and progress. StoriiCare offers flexible tools for scheduling, tracking, and completing Care Plan reviews using the Tasks feature.
📝 Care Plan may appear as Support Plan, Service Plan, Care & Support Plan, Know Me Better, etc., depending on your organization’s terminology.
Scheduling Options
Care Plan reviews can be scheduled directly through StoriiCare’s Tasks feature. This allows you to set up recurring review reminders at your preferred cadence — for example:
Monthly
Quarterly
Bi-annually
Annually
Tasks can be:
Created individually for specific participants, or
Scheduled in bulk for multiple participants at once, depending on your organization’s workflow.

Configuring Review Dates in Care Plans
To make Care Plan reviews even more structured, you can add review date fields directly to your Care Plan templates.
Navigate to your Business Settings.
Select the Care Plan tab.
(Note: both “Business Settings” and “Care Plan” terminology may appear differently in your account depending on customization.)
From here, administrators can:
Add date boxes such as Review Date or Next Review Date.
Mark these fields as required, ensuring staff cannot save a plan without setting a review schedule.
Completing a Care Plan Review from a Task
When it’s time to complete a scheduled review, staff can do so directly through their assigned Task.
Step-by-Step:
Go to the Tasks feature.
Locate the Care Plan Review Task.
If the Care Plan is hyperlinked within the task (as described in the linked guide), click the link to open the participant’s Care Plan.
Review and update each section as necessary.
Scroll down to Save your changes as you go.
Once updates are complete:
Click the back arrow inside the circle to return to the Tasks view.
Mark the task as complete by clicking the checkmark icon — it will turn green once finished.
Optionally, add notes to the task for context or audit purposes.
If needed, click Edit on the task to adjust the date or recurrence frequency for future reviews.

Use Tasks to ensure no Care Plans are overlooked during busy periods.
Encourage staff to include update notes when completing reviews for greater visibility into ongoing changes.
Remember that completed reviews remain part of your audit trail, providing accountability and transparency.
