How to Update an Existing Care Plan
Care Plans often need to be reviewed and updated as a participant’s needs, preferences, or goals change. Follow the steps below to update an existing Care Plan in StoriiCare.
Step 1: Open the Participant’s Profile
From your StoriiCare dashboard, search for the participant's name.
Click on their name to open their profile.
Step 2: Access the Care Plan Feature
Within the participant’s profile, click the Care Plan feature.
Depending on your organization’s terminology, this may appear as Support Plan, Service Plan, Know Me Better, or something similar.
You’ll now see a list of Care Plan categories.
Step 3: Select a Category and Locate the Care Plan
Click on the category tile that contains the Care Plan you wish to update.
Locate the specific Care Plan and click the arrow next to it.
This will expand the Care Plan so you can view it in full.
Step 4: Edit the Care Plan
Depending on your permission level, you should see a settings/cog wheel (⚙️) in the top-right corner of the Care Plan.
Click the cog wheel, then select Edit.
If you do not see the Edit option, you may not have permission to make changes. Contact your manager or supervisor for assistance.
Make the necessary updates or edits to the Care Plan fields.
Scroll to the bottom of the page and click Save to apply your changes.
Important: Understanding Historical Versions
When you edit a Care Plan, the previous version is overwritten — a full version history is not visible from the front end.
If you need to preserve the existing plan before making updates, you have two options:
Option 1: Mark the Care Plan as Historical
Before editing, you can mark the existing plan as “Historical” (terminology may vary depending on your organization’s setup). This keeps the original version accessible while you create a new, updated Care Plan.
Option 2: Add Update Notes
If you want to record smaller changes or progress updates, you can add Update Notes instead of editing the main plan. This maintains a visible log of ongoing adjustments without losing the original Care Plan content.
Best Practices
Always review the Care Plan carefully before editing to ensure accuracy.
Include review dates and notes to provide context for future updates.
Use the Update Notes section to track ongoing progress between major Care Plan revisions.
